Hi Bison,
a couple of things to test:
1 - leave the PHP box empty if you just want to see the debug info (just in case there's a syntax error in the PHP box which prevents the whole thing from displaying)
2 - ensure its published
3 - ensure it's set to a valid module position
4 - ensure it's set to ALL menu items.
When you install MetaMod or any other 3rd party module for the 1st time, it's easy to forget to set one of those things up.

to set the position for a module, click in the "module position" box and either:
1 - type in a position name. These position names will be different for each template so I can't tell you what they are on your site without seeing it in the backend.
2 - or click on the "select position" box and choose one of the options. Again, it needs to be a valid position for your template.
