that's very unusual indeed...

Can I ask you to check:

1 - can you take a look at the raw HTML of the page to see if the HTML for the flag is being output? Perhaps it's there but due to formatting differences it's being hidden?

If this is the case, then try adding a module class suffix to the MetaMod, and/or changing the "style for included modules". These affect the HTML that gets output by the MetaMod and may help. If you can't get a good combination of options there, then MetaMod Pro will completely remove any HTML output from the MetaMod, leaving only the normal HTML of the module it's including.

2 - try taking MetaMod out of the equation -- i.e. unpublish the MetaMod, then alter the settings of the languages module to make it appear again (i.e. publish it & set it to appear on some menus). Does the flag re-appear? If not, then MetaMod is not to blame.
